Frank, 19 years of age, was referred to the Transitional Youth program by his mother. He demonstrated classic signs of schizophrenia. Over the years, he had lived with various family members who housed him until they could no longer deal with his behavior. Frank had never been seen by a mental health service provider.

Treatment began and Frank was assisted in applying for Social Security benefits. He began to take medication and seemed to be doing well. However, one day he disappeared and it was learned that he had been sent to live with another relative. His counselor encouraged Frank's mother to bring him back for continued treatment.

Frank finally returned and the treatment process began again, including the application for SSI benefits, medication, housing, and most importantly, the development of trust. His counselor worked with Frank, his mother, his siblings, and with the Social Security office.

Today, Frank is receiving SSI and Medical benefits. He is living in the Center's Satellite Housing and will soon receive assistance in moving into permanent housing. He receives mental health services and medication, and his symptoms have markedly decreased. He has developed some friendships and is an active member at the Center's Clubhouse. Frank now wishes to obtain employment and will begin training in the Center's Valley Employment Services program. His progress demonstrates a truly seamless system that works.

Who Seeks Help From The Transitional Youth Program

Clients in this program are young adults, ages 17 & 21, who suffer from serious behavioral and emotional disorders and require assistance as they make the transition into early adulthood.

What Services Are Provided By The Transitional Youth Program

The program provides intensive case management, individual and group mental health counseling, psychiatric consultation, dual diagnosis counseling, medication monitoring, and crisis intervention. Clients are also assisted with access to community resources including education, transportation, housing, vocational rehabilitation, and socialization/recreational opportunities.

How The Transitional Youth Program Can Help

The Transitional Youth program provides active, community-based treatment to assist young adults in bridging the transition from adolescent to adult services. Clients are helped to lead full and vital lives through an effective combination of support, skill building, and resource linkage.

The program offers educational and vocational advancement opportunities. Clients learn independent living and coping skills and how to develop a natural support system in the community.
