TRANSITIONS

This program offers a creative and flexible continuum of care model to serve young adults, ages 18 to 21, who have been identified and authorized by the L.A. County Department of Mental Health for Adult Residential services or referred from sources approved by the Department. This model incorporates a variety of structured, therapeutic living environments with wrap around mental health, substance abuse, vocational, case management/brokerage and medication support services, specifically geared to address the multiple and diverse developmental issues confronting this group of young adults.
