David is a 23-year old male who has been hospitalized several times in the past five years. Prior to his last hospitalization, he had become increasingly withdrawn and reported hearing voices. ?Released from the hospital, he was referred to the Macdonald Carey East Valley Mental Health Center.
Upon admission to the Macdonald Carey Center, David was assigned a treatment team consisting of a clinician, rehabilitation specialist and psychiatrist. The team offered David a wide range of support services to the mutually agreed upon areas of difficulty that interfere with his productivity and independent community living. His treatment team also will serve as a liaison between him, his family, the hospital’s(as needed) and medical personnel. With help from his treatment team, David looks forward to reentering the community, finding a new apartment, learning to manage his time and money effectively and eventually looking for and maintaining a part-time job.
Michelle, a 37-year old woman, was referred to the Macdonald Carey East Valley Mental Health Center upon discharge from the hospital. Michelle had been hospitalized for depression after a serious suicide attempt. Since her release from the hospital, Michelle’s treatment team is working closely with her and her family to help Michelle to remain stable and out of the hospital.
